Traveler Information Form

Complete this form when requested by your travel agent. It is important that all names, birthdays, and genders match the government-issued identification used for travel. For international travel, we require the information as printed in each traveler’s passport. Please be aware that most countries and some tour operators require at least 6 months of passport validity beyond your return date.

Payment Forms

Credit Card Authorization Form

We use the credit card authorization form to collect payment authorizations for later processing with the vendor or vendors with whom we have booked your travel. Please note that it may take up to 48 hours to process your payment and have it reflected in your bank or credit card account.

E-Check Payment Authorization Form

A small number of select vendors accept ACH or e-check payments directly from your bank account. To use this payment type, you will need to be provided with a special password by your agent to proceed.

Princess® EZpay Agreement Form

Princess Cruises allows for this payment type. This payment method allows for regular, timed withdrawals of an irregular and sometimes surprising payment amount. As a result, we usually advise against this payment method.
